Enrollment

Before you enroll your child, we will need some information from you. If you have any questions, or to obtain copies of the following forms, please contact the office at office@osva.org or at 703-892-4846.

Registration

To register you must submit the following:

• A copy of child's Birth Certificate

• A Request for Student Permanent Record

• A current Virginia Health Form

• Copy of the child’s most recent report card.

• Copy of the child’s most recent standardized testing scores.

• If your child has an IEP or 504 Plan, it must be attached to the application.

• An email for a current teacher to fill out a recommendation form.

Other Information

Other helpful forms needed before the first day of school:

• Day Care Application

• Medication Consent Form

  • If at any time during the school year your child needs any prescription or over the counter medication a completed Medication Consent Form must be on file in the office.
